Dental AI Tools Compared: Imaging, Voice, Front Desk & Insurance AI (2026 Guide)

Dental AI Tools Compared: Imaging, Voice, Front Desk & Insurance AI (2026 Guide)

AI. It's all the rage. And you may be getting inundated with an inbox full of emails from an AI receptionist company, an AI x-ray company, an AI insurance-verification company, and probably two more you haven't opened yet. Every one of them says they'll transform your practice. None of them mention that they might not even integrate with your imaging software.

You don't have time to demo eleven vendors to find that out. So let's shortcut it.

What Most AI Pitches Skip

Over the last two years, the dental AI market sorted itself along distribution lines. VideaHealth powers the AI built into Dentrix ("Dentrix Detect AI"). Pearl is built into Eaglesoft's imaging and into Dentsply Sirona's DS Core. DEXIS ships its own AI inside DTX Studio.

The practice management and imaging software you already run has quietly narrowed your shortlist before you ever took a sales call. Before you compare features, it's worth checking what's already wired into your setup... then deciding whether a premium add-on like Overjet, or a value option like Denti.AI, earns its place on top of that.

There are four categories worth knowing, at a glance:

  • X-ray & imaging AI (~$49–$500+/mo per location): flags caries, bone loss and calculus on X-rays you already take, and turns findings into visuals patients understand.
  • Voice & clinical notes (free–$400/mo): hands-free perio charting and AI-written clinical notes. The whole game is whether it writes directly into your chart or leaves you copy-pasting.
  • Front office & phones (~$200–$1,200/mo): AI receptionists that answer 24/7 and book straight into your schedule, plus call analytics on what your team is missing.
  • Insurance & billing (~$225–$825/mo, or a percentage of collections): eligibility and full benefits breakdowns before the patient sits down, plus claims and AR follow-up behind the scenes.

Let's walk through each one... what they do, where they differ, and what to ask before you sign anything.

X-ray & Imaging AI

Every tool here detects caries, bone loss and calculus on your existing radiographs and overlays the findings chairside. Where they differ is FDA clearance depth, whether they live inside your imaging software or sit on top of it, and price.

Tool What makes it different Cost
Overjet The only AI cleared to measure disease (mm bone loss, caries outlines), not just flag it. Also powers claim review for most of the ten largest dental payers. ~$250–$500+/mo per location (reported)
Pearl — Second Opinion Most FDA clearances in the category. Native inside Eaglesoft Advanced Imaging and Dentsply Sirona's DS Core. ~$299/mo + setup (reported)
VideaHealth The engine behind Dentrix Detect AI. Only FDA-cleared pediatric algorithm (ages 3+). ~$79–$199/mo per chair (reported)
DEXIS DTXassist Built into the imaging software itself — no extra vendor. $2,388/yr (published)
Denti.AI Real FDA clearances at roughly 1/6th the premium price. The value option worth a look regardless of your platform. $49–$99/mo (published)

If you're on Dentrix, Videa's already sitting inside your software. If you're on Eaglesoft or run Dentsply Sirona equipment, that's Pearl.

Voice & Clinical Documentation AI

This category covers hands-free perio charting and AI-written clinical notes, so your hygienist doesn't need someone else calling out numbers. The difference is whether the data lands directly in your chart, or on your clipboard.

Tool What makes it different Writes into your chart?
Bola AI The integration leader — authorized, real-time write-back into six PMS platforms, distributed by both Henry Schein and Patterson. Yes, authorized
Dentrix Voice / Ascend Voice Built directly into the Dentrix Ascend chart by Henry Schein One — no third vendor. Yes, native (Dentrix only)
Open Dental built-in voice perio Command-based voice perio is included free in Open Dental's own perio module. Yes, native (Open Dental only), free
Pearl Voice New in April 2026 — ambient notes with 30+ procedure templates, a natural pairing if Pearl already runs your imaging AI. Unconfirmed by named PMS — ask
Denti.AI Voice Perio + Scribe Published pricing, a 10+ PMS compatibility list, positioned directly against Bola on price. Integrates, published

Ask every voice vendor one question before the demo ends — "show me it writes into my actual chart, not yours." Anything labeled "claimed" or "verify" in this category means the PMS-specific certification isn't public yet.

Front Office & Phone AI

These catch the calls your front desk can't, answering or texting back 24/7 and booking patients into open slots. They differ on voice agent vs. text agent, call analytics depth, and whether they replace your phone system outright.

Tool What makes it different Cost
Arini Pure-play AI phone receptionist with documented real-time schedule read/write through the official Dentrix API. Keeps your existing phones. ~$250–$1,200/mo (reported)
Weave AI Receptionist Owns the phone line itself, so AI, telephony and payments are one vendor. Early access targets Dentrix, Eaglesoft and Open Dental specifically. Not published (reported)
Peerlogic Call intelligence specialist — transcription and missed-opportunity detection, with a voice agent at the top tier. $199–$699/mo (published)
Viva AI Most transparent pricing in the category, 100+ languages with mid-call switching. $349–$1,199/mo + credits (published)

Test any front-office AI on your worst call type (an existing patient asking an insurance question) before you sign.

Insurance Verification & Billing AI

Everyone here is chasing the same two hours a day your team spends on payer portals and hold music. The real differentiator: do the deductible, maximum and frequencies land in your PMS coverage fields, or in a PDF your team re-keys?

Tool What makes it different Lands in your PMS fields?
Zuub The only vendor with documented field-level write-back into Open Dental, on 350+ carrier connections. Yes (Open Dental)
Overjet Verification Code-level detail — frequencies, downgrades, missing-tooth clauses — run 3+ days ahead of the schedule. Yes, named PMS support
DentalXChange Eligibility AI Rides the claim rails of a 35-year clearinghouse. Just won the largest DSO deployment in the category. Syncs
eAssist + Copilot The largest outsourced dental billing network, with AI that scrubs claims before a human submits. Rare fully-published pricing for a service. Works directly in your PMS

If a vendor can't answer "does it land in the field, or the PDF?" with a straight yes, budget for your team to keep re-keying data by hand.

Five Things to Know Before You Demo

  • "Integrates with your PMS" has four different meanings: native (built in), authorized integration (writes data directly), sync (reads a copy), and copy-paste (an overlay). Ask which one, every time.
  • Most pricing is quote-only. Treat any number you hear in a sales call as a starting point to negotiate from.
  • The big imaging AI vendors are becoming platforms. Overjet and Pearl both now sell imaging, voice and insurance verification as a suite. Ask about bundle pricing before buying piecemeal.
  • AI reads the same X-rays your insurer's AI reads. Overjet's engine also powers claim review for most of the ten largest dental payers. Documentation quality now matters on both sides of the claim.

Find Your Exact Stack in Two Clicks

Every combination above changes depending on whether you run Dentrix, Eaglesoft or Open Dental — and whether your imaging is DEXIS, CS Imaging, Eaglesoft Imaging, Romexis or Sidexis. Rather than make you cross-reference all of that by hand, we built an interactive tool: pick your practice management system and your imaging software, and it shows you the exact stack — imaging, voice, front office and insurance — that fights your setup the least.
